To schedule an appointment, please call 505-341-0000. Same day or next day appointment requests are given, based on availability. We can also place you on a waiting list to fill cancelled appointments.
pharmacy
Established patients may ask their pharmacy to fax refill requests to 505-341-1495. Faxed requests are reviewed Monday through Thursday; please allow 24 hours for processing.
medical records
Copies of medical records may be requested by submitting a medical record request form. There is an administrative fee for copying records, based on the Workers' Compensation fee schedule. Please allow 2 weeks for processing.
